Musical murmurs: an echo-phonocardiographic study.

K Venkataraman, R Siegel, S J Kim

    Summary

This study shows echocardiography can noninvasively pinpoint the source of musical murmurs in heart valve regurgitation. Echocardiography identified valve leaflet flutter matching murmur frequency, confirming its origin.

    Area of Science:

    • Cardiology
    • Medical Imaging
    • Acoustics

    Background:

    • Musical murmurs are rare heart sounds requiring precise localization for diagnosis.
    • Noninvasive diagnostic tools are crucial for evaluating cardiac conditions like valve regurgitation.

    Observation:

    • Five patients with musical murmurs underwent simultaneous echocardiography and phonocardiography.
    • Musical murmurs ranged from 40 to 158 cycles/sec, with one patient exhibiting an inspiratory honk due to tricuspid regurgitation.

    Findings:

    • Echocardiography revealed regular valve leaflet flutter (aortic, mitral, or tricuspid) in all patients.
    • The flutter frequency precisely matched the recorded musical murmur frequency.
    • This correlation confirms valve leaflet vibration as the source of musical murmurs.

    Implications:

    • Echocardiography is a valuable noninvasive method for identifying the origin of musical murmurs.
    • This technique aids in diagnosing and managing heart valve regurgitation.
    • Findings support the use of echo-phonocardiography in clinical cardiology for detailed murmur analysis.
