Malignant external otitis (MEO) treatment has improved with new antimicrobials and surgery. However, a subset of patients still face high mortality from this infection.

    Area of Science:

    • Otolaryngology
    • Infectious Diseases
    • Neurosurgery

    Background:

    • Malignant external otitis (MEO) is a severe infection of the external auditory canal.
    • Early diagnosis and treatment have improved outcomes, but mortality remains a concern in some patients.

    Observation:

    • A review of six MEO cases treated between 1976 and 1979 is presented.
    • Patients presented with symptoms ranging from severe ear pain to cranial nerve palsies.
    • Two high-risk patients with extensive skull base osteomyelitis were successfully treated.

    Findings:

    • Standard therapy involved aminoglycoside and carbenicillin combinations.
    • Aggressive treatment, including surgery and prolonged outpatient antibiotic therapy (tobramycin and carbenicillin), yielded excellent results in severe cases.

    Related Experiment Videos

  • Radionuclide bone scanning proved useful in monitoring treatment efficacy.
  • Implications:

    • Combined medical and surgical management, alongside advanced imaging, can improve outcomes for severe malignant external otitis.
    • This approach offers hope for patients with complex skull base infections.
    • Further research into MEO management strategies is warranted.
