    This study explored immune cell activity in lymph nodes of head and neck cancer patients. Immune cell activity correlated with tumor size and grade, suggesting a role in cancer progression.

    Background:

    • Cellular immune mechanisms in regional lymph nodes of head and neck cancer patients remain understudied.
    • Understanding these mechanisms is crucial for elucidating tumor-host interactions.

    Purpose of the Study:

    • To investigate the cellular immune mechanisms within regional lymph nodes of patients diagnosed with head and neck squamous cell carcinoma.
    • To assess the modulatory effects of lymph node cells on T-cell responses and their correlation with tumor characteristics.

    Main Methods:

    • Evaluation of 20 lymph nodes from eight patients with stage III-IV squamous cell carcinoma.
    • Utilized an in vitro culture system to assess the T-cell mitogenic response of peripheral blood mononuclear cells.
    • Modulation of T-cell response by adding cells from regional lymph nodes obtained during neck dissection.

    Main Results:

    • Modulatory activity, specifically augmentation, showed a significant correlation with primary tumor size.
    • Histopathologic grade of the tumor also demonstrated a significant correlation with the observed modulatory activity.
    • No correlation was found between modulatory activity and the histologic pattern of lymph node reactivity.

    Conclusions:

    • Regional immune responses in head and neck cancer patients may play a significant role in tumor-host interactions.
    • Further research is warranted to fully establish the biologic significance and prognostic implications of these regional immune mechanisms.
