    Area of Science:

    • Rheumatology
    • Dermatology
    • Vascular Surgery

    Background:

    • Scleroderma commonly presents with Raynaud's phenomenon.
    • Raynaud's phenomenon in scleroderma often leads to debilitating fingertip ulcers.

    Purpose of the Study:

    • To evaluate the incidence of digital ulcers in scleroderma patients with Raynaud's phenomenon.
    • To assess the efficacy of various treatments for digital ulcers in scleroderma.
    • To propose a management strategy for these ulcers.

    Main Methods:

    • Retrospective review of 59 scleroderma patients.
    • Analysis of clinical presentation, including Raynaud's phenomenon and digital ulcers.
    • Evaluation of medical management, sympathectomy, and surgical amputation outcomes.

    Main Results:

    • 93% of patients had Raynaud's phenomenon; 65% developed fingertip ulcers within 4 years.
    • Medical management and early sympathectomy showed limited success.
    • Conservative fingertip amputation was effective for nonhealing ulcers.

    Conclusions:

    • Digital ulcers are a common complication of Raynaud's phenomenon in scleroderma.
    • Conservative fingertip amputation is the preferred treatment for nonhealing digital ulcers.
    • A decision tree for managing digital ulcers in scleroderma is proposed.
