    Summary

    Systemic lupus erythematosus (SLE) can cause blistering skin conditions that mimic bullous pemphigoid (BP). Advanced microscopy revealed these lesions are a form of SLE, not BP, impacting diagnosis and treatment.

    Area of Science:

    Background:

    • Systemic lupus erythematosus (SLE) is an autoimmune disease with diverse clinical manifestations.
    • Bullous pemphigoid (BP) is a blistering disease characterized by autoantibodies against the dermal-epidermal junction.

    Observation:

    • Two patients with SLE developed blistering skin conditions resembling BP.
    • Direct immunofluorescence showed IgG and C3 deposition typical of BP.
    • Immunoelectron microscopy revealed IgG deposits below the basal lamina, not characteristic of BP.

    Findings:

    • The immunoelectron microscopy findings are consistent with SLE, ruling out BP.
    • Leukocyte attachment assays indicated functional immune complexes contributing to the bullous lesions.
    • This study is the first to use immunoelectron microscopy in suspected concurrent SLE and BP cases.

    Implications:

    • Findings challenge previous reports of concurrent SLE and BP.
    • Demonstrates a bullous form of SLE that clinically and immunopathologically mimics BP.
    • Highlights the importance of advanced diagnostic techniques for accurate disease classification.
