    Severe pyoderma gangrenosum can be effectively treated with cyclophosphamide, even without associated systemic disease. This immunosuppressive therapy offers a new option for patients unresponsive to other treatments.

    • Pyoderma gangrenosum (PG) is a rare, inflammatory neutrophilic dermatosis.
    • Often associated with systemic diseases, PG can be challenging to manage.

    Observation:

    • A 46-year-old woman presented with a 17-year history of severe, treatment-resistant pyoderma gangrenosum.
    • Her condition was refractory to systemic corticosteroids and sulfones.

    Findings:

    • Complete remission of pyoderma gangrenosum was achieved with 150 mg/day of cyclophosphamide.
    • Cyclophosphamide demonstrated significant efficacy in this recalcitrant case.

    Implications:

    • Immunosuppressive therapy, specifically cyclophosphamide, should be considered for severe, refractory pyoderma gangrenosum.
