    Summary

Systemic lupus erythematosus patients with silent lupus glomerulonephritis may have a benign renal course. Early biopsy and treatment are recommended for diffuse proliferative glomerulonephritis to improve outcomes.

    Area of Science:

    • Nephrology
    • Rheumatology
    • Immunology

    Background:

    • Diffuse lupus glomerulonephritis (DLG) can present without overt renal symptoms.
    • Previous work identified 12 such patients.

    Purpose of the Study:

    • To report the long-term renal outcomes of patients with DLG and initially silent nephropathy.
    • To evaluate the impact of biopsy findings on prognosis.

    Main Methods:

    • Follow-up of 10 patients with DLG and no initial renal evidence for 5-11 years.
    • Analysis of renal function, urinary abnormalities, and repeat kidney biopsies.

    Main Results:

    • Seven patients (Group I) experienced a benign renal course with stable function and mild/no urinary changes.

    Related Experiment Videos

  • Repeat biopsies in Group I showed significant resolution or improvement of lupus nephritis.
  • Three patients (Group II) had renal function deterioration, with one experiencing end-stage renal disease.
  • Conclusions:

    • Silent lupus nephritis may have a better renal prognosis, but requires larger studies.
    • Biopsy is recommended for SLE patients even without overt renal disease.
    • Treatment of diffuse proliferative glomerulonephritis is advised.
