    Summary

    Ketotic hypoglycemia (KH) is thought to stem from low alanine levels. However, this study found that low alanine levels result from hypoglycemia, not cause it, in fasting children.

    Background:

    • Ketotic hypoglycemia (KH) is often attributed to insufficient alanine for gluconeogenesis.
    • This hypothesis suggests alanine deficiency precedes and causes hypoglycemia.

    Purpose of the Study:

    • To investigate the relationship between glucose and alanine levels during fasting in children with and without KH.
    • To determine if alanine deficiency is a cause or consequence of hypoglycemia in KH.

    Main Methods:

    • Compared glucose and alanine concentrations in 18 fasting children with KH and 44 controls.
    • Analyzed the correlation and regression between glucose and alanine levels in both groups.

    Main Results:

    • Glucose levels dropped significantly more in children with KH compared to controls.
    • Alanine levels decreased in both groups, correlating significantly with glucose levels.
    • Regression analysis showed nearly identical glucose-alanine relationships, indicating alanine levels did not explain the greater glucose decline in KH.

    Conclusions:

    • Hypoalaninemia (low alanine) appears to be a consequence, not the cause, of hypoglycemia in ketotic hypoglycemia.
    • The study challenges the prevailing theory linking alanine shortage to the etiology of KH.
