    Background:

    • Hepatic portal venous gas (HPVG) is a rare radiographic finding.
    • It is often associated with serious intra-abdominal pathology.

    Observation:

    • Gas located at the liver periphery, within 2 cm of the capsule, suggests portal venous system involvement.
    • Branching radiolucencies within the liver parenchyma are indicative of HPVG.

    Findings:

    • HPVG is most commonly linked to necrotic bowel.
    • The presence of HPVG carries a very high mortality rate.

    Implications:

    • This radiographic sign is a critical indicator for emergency abdominal surgery.
    • Prompt surgical evaluation and intervention are crucial for patient survival.
