    Summary

    Congenital glaucoma in infants may stem from ectopic trabecular pillars compressing the eye's drainage angle. Goniotomy surgery appears to relieve this angle compression, restoring normal eye anatomy.

    Background:

    • Congenital glaucoma is a severe infant eye condition requiring prompt diagnosis and treatment.

    Observation:

    • Microscopic examination of eyes with congenital glaucoma revealed a unique anatomical abnormality.
    • A discontinuous membrane of ectopic trabecular pillars was observed bridging the drainage angle.

    Findings:

    • The ectopic trabecular pillars appeared to compress the trabecular meshwork, closing intertrabecular and transtrabecular spaces.
    • Goniotomy surgery successfully cleaved these aberrant pillars, altering the angle architecture to a non-glaucomatous neonatal state.
    • The uveal meshwork, corneoscleral meshwork, and Schlemm's canal were anatomically normal in the affected eyes.
