    Summary

    Painful bruising syndrome, characterized by recurrent bruising, is not caused by an autoimmune response to red blood cells. Studies suggest this condition is an artificially induced phenomenon, not a result of autosensitization.

    Background:

    • The painful bruising syndrome presents as chronic, relapsing ecchymoses in female patients.
    • Etiology is debated, with theories including sensitization to autologous erythrocytes or an artificial origin.

    Observation:

    • A 24-year-old woman diagnosed with painful bruising syndrome underwent intracutaneous testing.
    • Tests included autologous erythrocytes, homologous erythrocytes, erythrocyte stroma, and phosphatidyl-L-serine.
    • Reactions in the patient were inconsistent.

    Findings:

    • Control subjects without the syndrome could induce similar painful ecchymotic reactions.
    • These reactions in controls were achieved through manipulation of the tested skin areas.
    • The patient's reactions were inconsistent with autosensitization.

    Implications:

    • The findings challenge the hypothesis of autosensitization as the cause of painful bruising syndrome.
    • Evidence strongly suggests that painful bruising syndrome is an iatrogenic or self-inflicted phenomenon.
    • Further research should focus on identifying the specific mechanisms of artificial induction.
