    Periappendicitis, an uncommon appendicitis variant, can mimic left renal colic and cause ureteral obstruction. This case highlights a rare presentation of appendiceal inflammation leading to significant urologic complications.

    Background:

    • Periappendicitis is a histological diagnosis in approximately 5% of appendectomies for suspected acute appendicitis.
    • Appendicitis commonly mimics right ureteral colic, but left-sided presentations are exceedingly rare.
    • Complications of appendicitis can cause ureteral obstruction, typically on the right side.

    Observation:

    • This report details a pediatric case of periappendicitis presenting with clinical symptoms of left renal colic.
    • The patient exhibited partial left midureteral obstruction attributed to the appendiceal inflammation.
    • This represents a unique instance of appendiceal inflammation causing solitary left ureteral obstruction.

    Findings:

    • Periappendicitis presenting as left renal colic and partial left midureteral obstruction is unprecedented in medical literature.
    • This case demonstrates a novel association between appendiceal inflammation and isolated left-sided urologic compromise.
    • No prior reports document periappendicitis causing serious, solitary left ureteral complications.

    Implications:

    • Clinicians should consider appendiceal pathology in the differential diagnosis of left-sided renal colic and ureteral obstruction.
    • This case expands the known spectrum of urologic complications associated with appendicitis.
    • Awareness of this rare presentation may improve diagnostic accuracy and timely management of both appendicitis and ureteral obstruction.
