Background:

  • Neuroleptic malignant syndrome (NMS) is a rare but serious adverse reaction to neuroleptic medications.
  • It is characterized by mental status changes, muscular rigidity, hyperthermia, and autonomic instability.
  • Despite increased attention, etiopathogenesis and treatment require further investigation.

Observation:

  • This study prospectively observed 9 cases of NMS in psychiatric patients.
  • Seven cases met Levenson's diagnostic criteria, while two presented with milder symptoms.
  • Pre-syndrome mental status changes (e.g., stupor, coma, psychomotor excitement) were noted in 7/9 cases.

Findings:

  • NMS risk appears independent of neuroleptic class, D2 receptor affinity, or total dosage.
