Fibromyalgia (FM) is linked to impaired carbohydrate metabolism, showing higher pyruvate and lower lactate levels. These biochemical abnormalities suggest a need for targeted metabolic therapies in FM patients.

Background:

  • Primary fibromyalgia (FM) is a chronic pain condition.
  • Current treatments include analgesics and antidepressants.
  • FM has been associated with hyperpyruvicemia and reduced muscle high-energy phosphate.

Purpose of the Study:

  • To investigate potential carbohydrate metabolism impairments in FM patients.
  • To determine if FM is associated with biochemical abnormalities.

Main Methods:

  • Studied glycolysis in 25 FM patients, 10 hypothyroidism patients, 15 chronic pain patients, and 36 controls.
  • Performed laboratory tests on blood, erythrocytes, and plasma, including pyruvate, lactate, adenosine triphosphate, and enzyme activity.

Main Results:

  • Fibromyalgia and hypothyroidism patients showed increased pyruvate and decreased lactate production.
  • Adenosine triphosphate and lactate dehydrogenase isoenzymes were decreased specifically in FM patients.
  • Glycolysis was not significantly impaired in patients with osteoarticular chronic pain.

Conclusions:

  • Findings support that fibromyalgia is associated with biochemical abnormalities.
  • These abnormalities suggest a link to carbohydrate metabolism impairment.
  • Appropriate metabolic therapy may be required for fibromyalgia management.
