    Summary

Acute pyelonephritis in pregnancy is often caused by ureteral obstruction. Promptly treating bacteriuria and utilizing positional therapy, like the left lateral position, can prevent and manage this condition effectively.

    Area of Science:

    • Urology
    • Obstetrics
    • Nephrology

    Background:

    • Acute pyelonephritis is common during pregnancy, sometimes termed 'pyelitis of pregnancy'.
    • Existing literature often overlooks the obstructive etiology of this condition.

    Purpose of the Study:

    • To highlight the obstructive cause of acute pyelonephritis in pregnancy.
    • To emphasize the importance of addressing ureteral obstruction for effective management.

    Main Methods:

    • Review of existing literature and findings from studies in primates.
    • Investigating the role of bacterial introduction into the ureter with and without obstruction.

    Main Results:

    • Bacterial introduction into the ureter typically causes pyelonephritis only when ureteral obstruction is present.
    • Partial ureteral obstruction significantly increases the risk of acute pyelonephritis.
    • Vesicoureteral reflux is more common in pregnant primates.

    Conclusions:

    • Ureteral obstruction is a critical factor in the development of acute pyelonephritis during pregnancy.
    • Eradicating bacteriuria and understanding positional therapy are key for prevention and treatment.
    • The left lateral position can alleviate pain and aid treatment by relieving ureteral obstruction.
