    Summary

    A young woman recovered from sudden kidney failure and bleeding in her lungs, initially thought to be Goodpasture's syndrome. Later diagnosis revealed Wegener's granulomatosis, a rare autoimmune disease.

    Catheter-related bacteremia in hemodialysis patients: the role of the central venous catheter in prevention and therapy.

    Background:

    • Rapidly progressive renal failure and hemoptysis can present with overlapping symptoms.
    • Distinguishing between autoimmune conditions with similar initial presentations is clinically challenging.

    Observation:

    • A 25-year-old woman presented with fulminant, rapidly progressive renal failure and severe hemoptysis.
    • Initial clinical suspicion favored Goodpasture's syndrome due to the combination of renal and pulmonary hemorrhage.

    Findings:

    • The patient survived the acute episode of renal failure and hemoptysis.
    • Four years after the initial presentation, the patient was diagnosed with Wegener's granulomatosis (now known as Granulomatosis with Polyangiitis).

    Implications:

    • This case highlights the importance of considering a broader differential diagnosis in autoimmune diseases presenting with renal and pulmonary involvement.
    • Delayed diagnosis of Wegener's granulomatosis is possible even with severe initial symptoms.
    • Long-term follow-up is crucial for accurate diagnosis and management of complex autoimmune conditions.
