Summary

Large cysts in pleomorphic adenoma may arise from squamous metaplasia or enlarged ducts. Histopathological analysis of four cases suggests origins including tumor cell secretion, hemorrhage, or malignant tumor necrosis.

Area of Science:

  • Pathology
  • Oncology

Background:

  • Pleomorphic adenoma is the most common salivary gland tumor.
  • Large cyst formation within pleomorphic adenoma is a recognized but not fully understood phenomenon.

Purpose of the Study:

  • To investigate the histopathological origins of large cyst formation in pleomorphic adenoma.
  • To analyze four cases of pleomorphic adenoma with large cysts and review existing literature.

Main Methods:

  • Histopathological examination of four pleomorphic adenoma cases with large cysts.
  • Review of previously reported cases and relevant literature on cyst formation.

Main Results:

  • Observed cyst linings included keratinized squamous epithelium, squamous epithelium-like cells, atypical epithelial cells, and absence of epithelial lining.

Related Experiment Videos

  • One case presented as carcinoma in pleomorphic adenoma with cystic changes.
  • Hemorrhagic portions and necrosis were noted in cyst walls.
  • Conclusions:

    • Large cyst formation in pleomorphic adenoma may stem from squamous metaplasia of tumor cells.
    • Enlargement of duct-like structures due to excessive secretion, hemorrhage, or tumor necrosis are potential mechanisms.
    • Malignant transformation can also contribute to cystic changes.
