Pseudohyponatremia in multiple myeloma

Summary

Asymptomatic severe hyponatremia may indicate pseudohyponatremia due to chylomicronemia or paraproteinemia. Careful evaluation is crucial before treating hyponatremia, as normal sodium levels can mask hypernatremia in multiple myeloma patients.

Area of Science:

  • Clinical Chemistry
  • Internal Medicine
  • Nephrology

Background:

  • Severe hyponatremia in asymptomatic individuals necessitates careful clinical assessment.
  • Distinguishing true hyponatremia from pseudohyponatremia is critical for appropriate patient management.

Observation:

  • Chylomicronemia presents with creamy serum, while paraproteinemia is associated with increased serum viscosity.
  • In pseudohyponatremia, calculated serum osmolality is decreased, but measured osmolality remains within the normal range.

Findings:

  • Pseudohyponatremia can be associated with conditions like chylomicronemia and paraproteinemia.
  • Asymptomatic patients with normal serum sodium levels and multiple myeloma may have coexisting hypernatremia and hypertonicity.

Related Experiment Videos

Implications:

  • Clinicians must consider pseudohyponatremia in asymptomatic patients with severe hyponatremia.
  • Thorough diagnostic evaluation is essential to guide effective therapeutic interventions and avoid complications.
