Craniospinal arachnoid cyst: case report

Background:

Craniospinal arachnoid cysts are rare, often presenting with neurological deficits due to mass effect.
Early diagnosis and surgical management are crucial for favorable outcomes.

Observation:

A 54-year-old woman presented with headache, neck pain, and vomiting, exhibiting papilledema and hyperreflexia.
MRI revealed a large craniospinal arachnoid cyst compressing the cerebellum and upper spinal cord, causing obstructive hydrocephalus.

Findings:

Surgical decompression via suboccipital craniectomy and C1 laminectomy involved cyst wall resection and creation of a cyst-subarachnoid space communication.
Postoperatively, the cyst resolved, and the patient recovered fully without neurological deficits.
