    Summary

    Abnormal vascular reactions like white dermographism and delayed blanching occur in eczematous skin of atopic dermatitis and allergic contact dermatitis patients. These reactions in affected skin do not help diagnose atopic dermatitis.

    Background:

    • Atopic dermatitis (AD) and allergic contact dermatitis (ACD) involve complex skin inflammation.
    • Understanding vascular responses in these conditions is crucial for diagnosis and management.

    Purpose of the Study:

    • To investigate vascular reactions to mechanical and chemical stimuli in patients with AD and ACD.
    • To determine if abnormal vascular responses in affected skin can aid in diagnosing AD.

    Main Methods:

    • Examined vascular reactions to mechanical stroking, nicotinic acid ester, and methacholine chloride.
    • Assessed responses in both normal and eczematous skin areas of 100 AD patients and 20 ACD patients.

    Main Results:

    • White dermographism, nicotinic acid blanching, and delayed methacholine blanching consistently appeared in eczematous skin of both AD and ACD patients.
    • Normal skin in AD patients did not exhibit these abnormal vascular reactions.

    Conclusions:

    • The observed vascular reactions (white dermographism, nicotinic acid blanching, delayed methacholine blanching) in eczematous skin are secondary phenomena.
    • These vascular responses do not provide definitive diagnostic information for atopic dermatitis.
