In order to determine more accurately hypertensive cardiovascular risk, the term vascular overload has been proposed as an alternative to the conventional use of diastolic or systolic pressure. This term encompasses both static and pulsatile forces opposing flow, that is, arteriolar resistance, large-artery stiffness, and early wave reflection. This article explores the diagnostic and therapeutic usefulness of the vascular overload concept, especially as it pertains to geriatric hypertension.
