    Background:

    • Meconium staining of amniotic fluid is a common intrapartum event.
    • Accurate grading of meconium staining severity is crucial for determining appropriate neonatal management.

    Purpose of the Study:

    • To evaluate the accuracy and precision of midwives' visual assessment of meconium-stained amniotic fluid.
    • To determine if visual grading of meconium severity is a reliable indicator for clinical decision-making.

    Main Methods:

    • Four categories of meconium-stained amniotic fluid (clear, lightly, moderately, heavily stained) were prepared.
    • 20 midwives assessed the samples twice, resulting in 320 total assessments.
    • Inter-rater and intra-rater reliability were assessed using kappa statistics.

    Main Results:

    • No midwife achieved complete agreement with the standard assessment in over 85% of cases.
    • Excluding clear samples, midwives agreed on meconium-stained sample classification only 35.8% of the time.
    • Kappa statistics indicated poor agreement with the standard and low intra-rater reliability.

    • Visual assessment of meconium-stained amniotic fluid severity lacks sufficient accuracy and precision.
    • The current grading system is not a valid basis for differentiating care policies based on meconium staining severity.
    • Further research into objective assessment methods may be warranted.
