Anaphylactic reaction to cefazolin in pregnancy

Summary

Severe anaphylaxis during labor from cefazolin (CEZ) is rare but serious. Prompt treatment with ephedrine, glucocorticoids, and emergency C-section led to good maternal and fetal outcomes.

Background:

Anaphylaxis during pregnancy presents unique challenges due to physiological changes.
Cefazolin (CEZ), a common antibiotic, can rarely trigger severe allergic reactions.

Observation:

A case of severe anaphylaxis occurred in a pregnant patient receiving intravenous (i.v.) cefazolin during labor.
The patient experienced maternal shock and the fetus developed prolonged bradycardia.

Findings:

Immediate treatment involved ephedrine, glucocorticoids, and an emergency caesarean section.
Both mother and fetus recovered well following the intervention.

Implications:

This case highlights the importance of recognizing and managing rare anaphylactic events in obstetric patients.
Aggressive management can lead to favorable maternal and fetal outcomes even in severe reactions.
Awareness of potential antibiotic-induced anaphylaxis is crucial in labor and delivery settings.
