Summary

Surgery for low-grade supratentorial gliomas can effectively control seizures in most patients. Tumor resection, often with adjuvant therapy, led to seizure freedom in two-thirds of cases, suggesting the tumor

Area of Science:

  • Neurosurgery
  • Neurology
  • Oncology

Background:

  • Low-grade supratentorial gliomas frequently present with seizures.
  • Seizures associated with these tumors are often refractory to medical management.

Purpose of the Study:

  • To evaluate the efficacy of surgical resection in controlling seizures in patients with low-grade supratentorial gliomas.
  • To investigate the role of tumor infiltration in epileptogenesis.

Main Methods:

  • Retrospective analysis of 52 patients who underwent surgery for low-grade supratentorial gliomas between 1991-1993.
  • Tumor resection without intraoperative electrocorticography.
  • Adjuvant radiotherapy and chemotherapy in select cases.

Main Results:

Related Experiment Videos

  • 37 patients presented with seizures, often drug-refractory.
  • Postoperative seizure freedom was achieved in approximately two-thirds of the 33 surviving patients (mean follow-up 28 months).
  • Histological analysis suggested peripheral tumor infiltration may contribute to epileptogenesis.

Conclusions:

  • Surgical resection is an effective treatment for seizure control in patients with low-grade supratentorial gliomas.
  • The peripheral zone of cortical tumor infiltration may play a role in seizure development.
  • Further research into the mechanisms of epileptogenesis in gliomas is warranted.
