Background:

Congenital malformations require accurate prenatal and postnatal diagnosis.
Extra-lobar intra-abdominal pulmonary sequestration is a rare condition presenting as a fetal abdominal mass.

Observation:

A 20-week fetus presented with a left upper quadrant abdominal mass.
Imaging studies including ultrasonography, CT, and MRI confirmed a homogenous echogenic mass in the left diaphragmatic crus.

Findings:

Histopathology revealed alveolar and bronchial tissue with a visceral pleural covering, confirming extra-lobar intra-abdominal pulmonary sequestration.
The mass exhibited respiratory movement on imaging.
