Hepatocellular carcinoma recurrence after liver transplant is common. A new protocol using a hepatotoxin, pyrazofurin, significantly reduced metastatic disease in a rat model, offering a promising strategy.

Area of Science:

  • Hepatobiliary surgery
  • Oncology
  • Transplantation immunology

Background:

  • Recurrent hepatocellular carcinoma (HCC) after liver transplantation (LT) is a significant clinical challenge, often attributed to undetected micrometastases.
  • Current strategies lack effective methods to eliminate these micrometastases before or during transplantation.

Purpose of the Study:

  • To develop and evaluate a novel treatment protocol aimed at preventing HCC recurrence by targeting micrometastatic disease.
  • To assess the efficacy of a hepatotoxin, pyrazofurin, in a rat model of HCC micrometastasis prior to liver transplantation.

Main Methods:

  • A rat model was established by intravenously injecting Morris hepatoma cells to simulate micrometastatic HCC.
  • Three groups were studied: no treatment, 5% dextrose in water (D5W) followed by orthotopic liver transplant (OLTX), and pyrazofurin treatment followed by OLTX.

Related Experiment Videos

  • Pulmonary metastases were quantified on Day 35 post-transplantation.
  • Main Results:

    • The pyrazofurin-treated group showed a significant reduction in pulmonary nodules compared to control groups (4.8 nodules/animal vs. 45.2 and 60.8 nodules/animal).
    • Pyrazofurin effectively prevented hematogenous spread and micrometastasis of hepatoma cells in this model.
    • The model proved reliable for studying hepatoma metastasis.

    Conclusions:

    • This study demonstrates the potential of pyrazofurin as a neoadjuvant therapy to reduce HCC micrometastases before liver transplantation.
    • The developed rat model provides a valuable platform for further investigation of novel therapeutic strategies, including different hepatotoxins and transplantation protocols.
