Background:

Tracheopathia osteochondroplastica (TO) is a rare, benign condition characterized by the formation of osteocartilaginous nodules within the tracheal wall.
Patients often present with nonspecific respiratory symptoms, leading to delayed diagnosis.

Observation:

This case highlights TO as an unexpected finding during fiberoptic bronchoscopy in a patient with recurrent respiratory infections and hemoptysis.
The diagnostic challenge is compounded by the potential for severe bleeding during the procedure.

Findings:

Fiberoptic bronchoscopy revealed tracheopathia osteochondroplastica, an uncommon cause of airway obstruction.
Massive hemoptysis was identified as a significant complication during the bronchoscopic examination.
