Summary

A rare peripheral odontogenic fibroma case in a child showed benign behavior despite cellular stroma. Cartilaginous differentiation in this odontogenic fibroma is a novel finding.

Background:

Peripheral odontogenic fibroma (POF) is a rare benign tumor originating from the periodontal ligament.
These tumors typically present as asymptomatic gingival enlargements.
Understanding POF behavior, especially in pediatric patients, is crucial for diagnosis and management.

Observation:

A unique case of POF in a 3-year-old female child is presented.
The tumor manifested as a gingival swelling extending to the hard palate.
Histopathological examination revealed a cellular stroma and lack of encapsulation.

Findings:

Despite a highly cellular stroma and absence of encapsulation, the tumor exhibited benign behavior.
The most significant finding was the presence of cartilaginous differentiation within the tumor stroma.
This specific feature, cartilaginous differentiation in POF, has not been previously documented in English literature.

Implications:

This case expands the known histopathological spectrum of peripheral odontogenic fibroma.
The presence of cartilaginous differentiation suggests a complex mesenchymal origin or differentiation potential.
Further research is warranted to understand the behavior and origin of such variants in pediatric odontogenic tumors.
