    Summary

    Liver granulomas in sarcoidosis are uncommon, found in 24% of needle biopsies. Extensive erythema nodosum may show pathological findings, but hepatic granulomas correlate poorly with other clinical markers.

    Background:

    • Sarcoidosis is a multisystem inflammatory disease.
    • Liver involvement in sarcoidosis can manifest as granulomas.
    • Erythema nodosum is a common manifestation of sarcoidosis.

    Purpose of the Study:

    • To investigate the prevalence and correlations of hepatic granulomas in sarcoidosis.
    • To assess liver function in sarcoidosis patients.
    • To explore the relationship between erythema nodosum and liver pathology in sarcoidosis.

    Main Methods:

    • Needle biopsies of the liver in 121 sarcoidosis cases.
    • Liver function tests (LFTs) in 325 sarcoidosis and 132 non-sarcoid erythema nodosum patients.
    • Correlation analysis of hepatic granulomas with clinical and laboratory findings.

    Main Results:

    • Granulomas compatible with sarcoidosis were found in 24% of liver biopsies.
    • Pathological liver findings were more frequent in patients with extensive erythema nodosum.
    • Hepatic granulomas were more common in patients with parenchymal lung sarcoidosis compared to those with only bilateral hilar adenitis.
    • No significant correlations were found between hepatic granulomas and other clinical or laboratory parameters.

    Conclusions:

    • Hepatic granulomas are present in a minority of sarcoidosis patients.
    • The presence of erythema nodosum, particularly extensive forms, may be associated with liver pathology.
    • Sarcoidosis in Scandinavian countries appears to have a favorable prognosis with lower rates of liver involvement.
    • Further research is needed to elucidate the specific mechanisms and clinical significance of liver granulomas in sarcoidosis.
