Background:

Mayo Clinic utilized a traditional paper-based system for patient history management.
A prior system defined 'episodes of care' as 'registrations'.

Purpose of the Study:

To implement clinically defined episodes of care for improved patient management.
To introduce an episode-based summary list of patient problems.
To establish a foundation for the electronic medical record (EMR).

Main Methods:

A new clinical definition of 'episode of care' was adopted in November 1996.
A computer-generated 'Master Sheet Summary Report' organized by episode was integrated into patient histories.
Online access to the episode-based problem summary was provided via EMR-capable workstations in 1997.

Main Results:

The implementation facilitated improved care management and operational processes.
A standardized, clinically relevant patient problem summary was generated.
Enhanced access to patient information was provided across the healthcare system.

Conclusions:

The new episode-based system provides a robust construct for the electronic medical record.
