Summary

A hemodialysis patient with anemia resistant to erythropoietin (EPO) improved after treating hyperthyroidism. This suggests hyperthyroidism can cause EPO-resistant anemia, which is reversible with proper treatment.

Area of Science:

  • Nephrology
  • Endocrinology

Background:

  • Erythropoietin (EPO) resistance is a challenge in managing anemia for hemodialysis patients.
  • Primary hyperthyroidism can present with complex hematological manifestations.

Observation:

  • A 26-year-old male hemodialysis patient exhibited EPO-resistant anemia.
  • The patient's condition was associated with primary hyperthyroidism.

Findings:

  • Treatment with methimazole resolved the patient's hyperthyroidism.
  • Anemia improved significantly, and the need for blood transfusions decreased after hyperthyroid treatment.
  • Hematocrit levels normalized without EPO therapy or transfusions post-treatment.

Implications:

  • Primary hyperthyroidism is a potential cause of EPO-resistant anemia in hemodialysis patients.
