Summary

Right-sided strokes can cause visuo-spatial neglect. EEG data suggests that increased delta brainwave activity in posterior regions, particularly an imbalance between hemispheres, may underlie this attention disorder.

Background:

  • Right-sided capsulo-lenticular strokes are associated with left visuo-spatial neglect.

Purpose of the Study:

  • To investigate the neural mechanisms of visuo-spatial neglect following capsulo-lenticular strokes.
  • To compare EEG activity in patients with and without neglect.

Main Methods:

  • Electroencephalography (EEG) cartography with 20 channels was used.
  • Analyzed absolute delta and theta amplitude peaks and interhemispheric ratios in defined regions of interest.
  • Compared EEG data between 33 patients with capsulo-lenticular stroke, stratified by presence (n=16) or absence (n=17) of neglect.

Main Results:

  • Patients with neglect exhibited higher delta activity compared to those without neglect.
  • The left-to-right ratio of delta activity in posterior homologous regions was reduced in neglect patients.
  • Delta activity differences between groups were not site-dependent.

Conclusions:

  • Right posterior cortical dysfunction, leading to interhemispheric imbalance, may contribute to visuo-spatial neglect.
  • EEG delta activity patterns can differentiate between neglect and non-neglect patients post-stroke.
