Selective foeticide using YAG-laser successfully treated twin-to-twin transfusion syndrome in monochorionic twins. This intervention resulted in a healthy birth and normal psychomotor development at six months.

Area of Science:

  • Perinatology
  • Maternal-Fetal Medicine
  • Fetal Surgery

Background:

  • Twin-to-twin transfusion syndrome (TTTS) is a serious complication in monochorionic diamniotic pregnancies.
  • Early diagnosis via ultrasound is crucial for timely intervention.
  • Hydrocephalus in the donor twin indicates severe disease progression.

Observation:

  • Ultrasound identified TTTS in monochorionic diamniotic twins at 20 weeks gestation.
  • The donor twin exhibited hydrocephalus, while the recipient twin was unaffected.
  • Selective foeticide was performed by laser coagulation of the donor's umbilical cord vessels.

Findings:

  • The pregnancy proceeded without complications following the procedure.
  • A healthy female infant was delivered at 34 weeks gestation.

Related Experiment Videos

  • The infant had a birth weight of 2935 g and normal psychomotor development at six months.
  • Implications:

    • Selective foeticide is an effective treatment for severe TTTS with fetal anomalies.
    • Laser coagulation of umbilical vessels offers a viable option for managing TTTS.
    • This case highlights the potential for successful pregnancy outcomes despite complex fetal conditions.
