Area of Science:

  • Cardiology
  • Thoracic Surgery
  • Infectious Diseases

Background:

  • Chronic constrictive pericarditis significantly impairs cardiac function and quality of life.
  • Surgical pericardiectomy is the definitive treatment for constrictive pericarditis.
  • Identifying optimal surgical approaches and outcomes is crucial for patient management.

Purpose of the Study:

  • To evaluate the causes, surgical techniques, and outcomes of pericardiectomy for chronic constrictive pericarditis.
  • To assess the early postoperative mortality and morbidity rates.
  • To determine the improvement in functional capacity after surgery.

Main Methods:

  • Retrospective review of 42 patients undergoing surgery for chronic constrictive pericarditis (January 1994 - January 2002).

Related Experiment Videos

  • Median sternotomy approach with complete anterior pericardium resection.
  • Cardiopulmonary bypass used selectively for additional cardiac procedures.
  • Main Results:

    • Tuberculosis was the most common cause (57.1%).
    • Early postoperative mortality was 11.9% (5 patients).
    • Dramatic improvement in functional capacity observed within the first postoperative month, with a significant shift from severe to mild/no functional limitation.

    Conclusions:

    • Median sternotomy is a recommended surgical approach for chronic constrictive pericarditis.
    • Cardiopulmonary bypass is safe and effective when indicated.
    • Surgical pericardiectomy leads to substantial functional recovery in patients with constrictive pericarditis.
