Exercise stress testing, commonly known as a treadmill test, is a noninvasive procedure used to evaluate cardiovascular function and diagnose heart conditions.
Definition
An exercise stress test measures the heart's response to exertion using a treadmill or stationary bicycle. Chest electrodes record the heart's electrical activity through an ECG, and blood pressure is monitored regularly.

Background:

  • Tropicamide is an anticholinergic medication commonly used for ophthalmic examinations.
  • Its potential systemic effects, particularly on cardiovascular parameters during physiological stress, are not fully understood.

Purpose of the Study:

  • To evaluate the impact of tropicamide 0.5% eye drops on cardiovascular parameters during exercise testing in healthy individuals.
  • To determine if pupil dilation influences these cardiovascular responses.

Main Methods:

  • 154 healthy subjects underwent exercise testing, divided into three groups based on pupil dilation (not dilated, partially dilated, widely dilated).
  • A control group of 66 healthy individuals not receiving tropicamide was included for comparison.
  • Resting and exercise cardiovascular parameters (heart rate, blood pressure) were measured and compared across groups.

Main Results:

  • Tropicamide affected resting and exercise cardiovascular parameters in subjects with non-dilated and partially dilated pupils.
  • Subjects with widely dilated pupils showed results similar to controls.
  • The study group, as a whole, achieved higher workloads than controls (126 vs. 119 W).

Conclusions:

  • Ocular tropicamide instillation demonstrably affects cardiovascular parameters at rest and during exercise.
  • A lower heart rate was observed at initial exercise levels in subjects using tropicamide.
  • Tropicamide does not alter the ability to reach target heart rate or affect overall stress test outcomes at symptom-limited levels.
