Laser intrastromal keratoplasty successfully corrected high hyperopia and irregular astigmatism in a patient with a thin cornea. This innovative procedure offers a potential solution for complex refractive errors previously considered untreatable.

Findings:
- Postoperatively, best spectacle-corrected visual acuity improved to 20/50, with a significant reduction in hyperopia and astigmatism.
- Corneal topography demonstrated a smoother surface and increased curvature, while central corneal thickness increased to 600 µm.
Implications:
- Laser intrastromal keratoplasty is a feasible technique for correcting high hyperopia and irregular astigmatism in eyes with thin corneal beds.
- This procedure may offer a viable alternative for patients unsuitable for conventional refractive surgeries.
