Summary

Hyperbaric Oxygen Therapy (HBOT) shows no consistent evidence of benefit for Multiple Sclerosis (MS) treatment. Isolated findings of improvement require further investigation in well-designed trials before routine use is considered.

Background:

  • Multiple Sclerosis (MS) is a chronic, progressive neurological disease with no known cure.
  • Hyperbaric Oxygen Therapy (HBOT) has been speculatively proposed to potentially slow or reverse MS progression.

Purpose of the Study:

  • To systematically evaluate the efficacy and safety of Hyperbaric Oxygen Therapy (HBOT) as a treatment for Multiple Sclerosis (MS).

Main Methods:

  • Comprehensive literature search of multiple databases including Cochrane MS Group, Cochrane Central Register, MEDLINE, and NLM.
  • Inclusion of all randomized controlled trials comparing HBOT with sham therapy in MS patients.
  • Independent data extraction and methodological quality assessment by two reviewers.

Main Results:

  • Ten reports from nine trials (504 participants) met the selection criteria.
  • Two trials suggested positive results, while seven found no evidence of treatment effect.
  • A statistically significant improvement in the Expanded Disability Status Scale (EDSS) was observed in one analysis of the HBOT group (-0.85 points at 12 months).

Conclusions:

  • No consistent evidence supports the beneficial effect of HBOT for MS treatment.
  • The routine use of HBOT for MS is not currently justified.
  • Further well-designed trials are needed to confirm isolated suggestive findings, but are not warranted by this review.
