Radical resection of large retroperitoneal liposarcomas, even those progressing during chemotherapy, can lead to successful outcomes. Multiorgan surgery and specialized sarcoma unit management are crucial for these challenging tumors.

Area of Science:

  • Oncology
  • Surgical Oncology
  • Gastrointestinal Surgery

Background:

  • Retroperitoneal sarcomas are rare, often asymptomatic tumors that can grow significantly before detection.
  • Liposarcomas represent a common subtype of soft tissue sarcomas, frequently occurring in the retroperitoneum.

Observation:

  • Two cases of large, unresectable liposarcomas (40x40 cm and 19x15 cm) are presented, one with progression during chemotherapy.
  • Both patients underwent radical surgical resection, involving multiorgan extirpation in one case (pancreatic tail, spleen, transverse colon).

Findings:

  • Both patients achieved complete recovery without complications following surgical intervention.
  • Disease-free survival was confirmed at two years post-operation for both individuals.

Implications:

  • Radical resection, despite the complexity and potential need for multiorgan surgery, can be a curative option for retroperitoneal liposarcomas.
  • Centralized management within a national Sarcoma Unit is vital for optimizing patient outcomes and prognosis.
