Summary

Extranodal non-Hodgkin's lymphoma (NHL) is increasingly common, particularly in individuals of Arabic descent. Aggressive extranodal NHL, often diffuse large B-cell lymphoma affecting the gastrointestinal tract, is associated with poorer survival rates compared to early nodal disease.

Background:

Purpose of the Study:

  • To investigate the epidemiology of NHL in the Middle East.
  • To compare clinical characteristics and treatment outcomes of primary nodal versus extranodal NHL.

Main Methods:

  • Retrospective analysis of 422 NHL cases diagnosed between 1998 and 2003.
  • Classification into primary nodal and primary extranodal NHL based on strict criteria.
  • Comparison of clinical features and outcomes between extranodal and early-stage nodal cases.

Main Results:

  • Extranodal NHL constituted 46% of all diagnosed cases.
  • A higher prevalence of nodal NHL was observed in Arab populations compared to Asians.
  • Diffuse large B-cell lymphoma was the most common subtype in extranodal cases (71%), frequently involving the gastrointestinal tract (45%).
  • Primary nodal high-grade lymphomas had a significantly better overall survival than primary extranodal disease (p=0.003).
