Scopolamine butylbromide (SB) can trigger severe headaches resembling migraine without aura in individuals with a history of migraine. Caution is advised when using SB, as it may worsen existing migraines.

- Only one non-migraineur reported a mild headache after SB injection, suggesting a predisposition in migraineurs.

- SB can trigger severe migraine-like headaches or exacerbate pre-existing migraines.
- SB-induced headaches may result from an imbalance between acetylcholine and other neuropeptides.
- Increased caution is recommended when administering SB due to its potential to aggravate migraines.
