Background:

  • Enlarged pituitary gland mimicking adenoma in a pediatric patient.
  • Acute hydrocephalus leading to partial empty sella formation on MRI.

Observation:

  • The pituitary gland initially appeared enlarged, resembling an adenoma.
  • Following an acute hydrocephalus event, a partial empty sella was observed on Magnetic Resonance Imaging (MRI).

Findings:

  • Pituitary gland volume normalized and refilled the sellar fossa after cerebrospinal fluid (CSF) shunt replacement and normalization of intracranial pressure.
  • Suggests ischemic atrophy of the adenohypophysis may contribute to primary empty sella in chronic raised intracranial pressure, hindering gland recovery.

Implications:

  • Restitution of the empty sella may serve as an indicator of normalized intracranial pressure in specific pediatric cases.
  • Highlights the dynamic relationship between intracranial pressure, pituitary gland volume, and sellar morphology.
