Chronic obstructive pulmonary disease (COPD) severity is independently linked to both emphysema and small airways disease. These findings confirm that both conditions contribute to airflow obstruction in COPD patients.

- Both emphysema extent and small airways disease independently predict COPD severity and airflow obstruction.
- This study provides combined evidence for the distinct roles of emphysema and small airways disease in COPD pathophysiology.
