Background:

  • Complex percutaneous coronary intervention (PCI) often necessitates large-lumen guiding catheters.
  • Vascular access site disease or tortuous anatomy can restrict guiding catheter insertion, complicating PCI procedures.

Observation:

  • A unique double guiding catheter technique was employed in two patients requiring complex PCI.
  • Two 6F guiding catheters were concurrently used to engage the target vessel ostium.

Findings:

  • In one case, the double catheter setup facilitated kissing-balloon dilation after single-stent placement.
  • In the second case, sequential stent delivery and simultaneous kissing stent deployment were achieved, followed by high-pressure kissing-balloon postdilation.

Implications:

  • This double guiding catheter technique offers a viable solution for complex PCI when conventional methods are limited by anatomical challenges.
  • It expands treatment options for patients with severe coronary artery disease requiring advanced revascularization strategies.
