Observation:
- A previously healthy 20-year-old female presented with fever, jaundice, and altered sensorium during a Dengue outbreak.
- Laboratory results indicated Dengue IgM positivity and severe acute liver injury.
- Biochemical markers showed markedly elevated liver enzymes (ALT, AST), high bilirubin, prolonged PT INR, and thrombocytopenia.
Findings:
- The patient was diagnosed with Dengue-associated Acute Liver Failure (ALF).
- Other potential causes of acute hepatic failure were excluded.
- Supportive care and platelet transfusion led to significant clinical and biochemical recovery.
Implications:
- This case underscores the importance of considering Dengue as a potential cause of ALF, even in non-endemic areas or during outbreaks.
- Early recognition and management of Dengue-induced ALF are crucial for patient outcomes.
- Further research into the mechanisms of Dengue hepatotoxicity is warranted.
