Summary

Background:

Primary mesenteric liposarcoma is an exceptionally rare malignancy.
Aggressive surgical resection with wide margins is the standard treatment for localized disease.

Observation:

A 52-year-old male presented with significant abdominal distension due to a large, slow-growing mesenteric liposarcoma.
Pre-operative imaging revealed a massive, encapsulated abdominal tumor without distant metastases or ascites.

Findings:

The patient underwent successful surgical resection of a 22-kilogram well-differentiated liposarcoma.
Histopathology confirmed the diagnosis with rare mitoses.
Adjuvant chemotherapy was administered for eight cycles.

Implications:

This case underscores the efficacy of radical surgery and adjuvant therapy in managing large primary mesenteric liposarcomas.
Long-term follow-up demonstrated no evidence of recurrence or metastasis, supporting the chosen treatment strategy.
