Eosinophilic peritonitis (EP) is a rare complication of peritoneal dialysis. This case study shows successful management of EP with loratidine after bacterial peritonitis treatment.

- A middle-aged male on continuous ambulatory peritoneal dialysis (CAPD) received intraperitoneal streptokinase for inflow/outflow issues.
- Post-streptokinase, Staphylococcus aureus peritonitis developed, followed by persistent turbid effluent despite antibiotic treatment.
Findings:
- Effluent analysis revealed a predominance of eosinophils, leading to an EP diagnosis.
- The patient's EP resolved completely after treatment with the antihistamine loratidine.
Implications:
- This case highlights a potential link between intraperitoneal streptokinase and EP in CAPD patients.
- Antihistamine therapy, such as loratidine, may be an effective treatment for drug-induced EP.
