Conserved Telomere Length in Human Ectopic Thyroids: An Argument Against Premature Differentiation Causing Arrested

Premature differentiation does not cause ectopic sublingual thyroid glands. Telomere length (TL) was similar in ectopic and normal thyroid tissues, and telomerase reverse transcriptase (hTERT) expression was not elevated.

Background:
- The cause of ectopic sublingual thyroid glands, resulting from arrested migration of the thyroid anlage, is unknown.
- These glands have normal architecture but insufficient growth, often causing congenital hypothyroidism.
- A hypothesis suggested premature differentiation, indicated by decreased telomere length (TL), as a cause.

- Absolute telomere length (TL) was measured using quantitative polymerase chain reaction (qPCR) on genomic DNA.
- Telomerase reverse transcriptase (hTERT) expression was quantified via qPCR on total RNA.
- Measurements were performed on four ectopic and six orthotopic thyroid samples.

- Mean TL was 140.45±40.07 kb/diploid genome in ectopic thyroids versus 97.50±30.48 kb/diploid genome in orthotopic thyroids (p=0.12).
- Telomere length (TL) shortening was not observed in ectopic thyroid tissues compared to orthotopic ones.
- Telomerase reverse transcriptase (hTERT) expression was quiescent (not detected) in both ectopic and orthotopic thyroid tissues.
Conclusions:
- Premature differentiation is unlikely to be the cause of arrested thyroid migration leading to ectopic sublingual glands.
- The absence of TL shortening and compensatory hTERT expression suggests arrested migration is not linked to premature differentiation.
- Ectopic thyroids are indirectly suggested to not have a higher cancer risk than orthotopic thyroids.
