Summary

Needleless and needle-assisted submucosal injections show similar tissue elevation efficacy in GI endoscopy. More viscous solutions, however, provide higher and more persistent elevations compared to normal saline.

Background:

  • Submucosal injection is crucial in gastrointestinal (GI) endoscopy for reducing resection risks and enabling submucosal dissection.
  • Current methods typically involve needle-assisted or needleless injection devices.

Purpose of the Study:

  • To compare the efficacy of needle-assisted versus needleless submucosal injection techniques.
  • To evaluate the impact of substance viscosity on submucosal tissue elevation.

Main Methods:

  • An ex vivo porcine model was used to compare needle and needleless injection techniques.
  • Five different substances with varying viscosities were injected.
  • Submucosal elevation height was measured at three time points (t0, t1, t2), and solution viscosity was recorded.

Main Results:

  • No significant difference in submucosal elevation height or persistence was observed between needleless and needle-assisted techniques in the stomach or rectum (except for initial rectal elevation).
  • More viscous solutions consistently resulted in significantly higher and more persistent tissue elevations compared to normal saline, irrespective of the injection method.
  • The initial elevation in the rectum was slightly higher with the needle-assisted technique but not clinically significant.

Conclusions:

  • Needleless and needle-assisted submucosal injection techniques demonstrate comparable efficacy in achieving and maintaining tissue elevation.
  • Substance viscosity is a key factor influencing the height and duration of submucosal elevations.
  • The findings suggest that needleless devices are a viable alternative to needle-assisted methods for submucosal injections.
