Urethroplasty effectively treats complicated anterior urethral strictures in patients who failed other procedures. This surgical repair significantly improved urine flow (Qmax) and showed no recurrence in a 55-month follow-up.
Area of Science:
- Urology
- Surgical Innovation
Background:
- Anterior urethral strictures often require complex surgical intervention.
- Previous treatments like urethral dilatation or internal urethrotomy can fail, necessitating advanced procedures.
Purpose of the Study:
- To evaluate the efficacy and outcomes of urethroplasty for complicated anterior urethral strictures.
- To compare surgical outcomes following various urethroplasty techniques.
Main Methods:
- Twelve patients (including 3 boys) with refractory anterior urethral strictures underwent urethroplasty.
- Surgical techniques included end-to-end anastomosis, augmented anastomotic urethroplasty, onlay urethroplasty (preputial flap or buccal mucosal graft), and tubed urethroplasty (Ducket procedure).
- Pre- and post-operative maximal urinary flow rate (Qmax) and surgical outcomes were assessed.
Main Results:
- All 12 patients experienced improved post-operative Qmax.
- Various urethroplasty techniques were employed, including grafts and flaps.
- No major complications or recurrences were observed during a mean follow-up of 55 months (range 17-102 months).
Conclusions:
- Urethroplasty is a highly effective therapeutic option for complicated anterior urethral strictures.
- The procedure demonstrates durable outcomes with significant improvement in urinary flow.
- Urethroplasty offers a successful treatment pathway for patients with treatment-resistant urethral strictures.
