Three-dimensional endoscopic ultrasonography (3D-EUS) significantly improves rectal cancer staging accuracy compared to conventional methods. This advanced technique offers higher precision in T and N staging, aiding in better preoperative assessment for rectal cancer patients.
Area of Science:
- Gastroenterology
- Oncology
- Medical Imaging
Background:
- Conventional endoscopic ultrasonography (EUS) for rectal cancer staging has limitations.
- Rigid probes and standard echoendoscopes present challenges in accurate tumor assessment.
Purpose of the Study:
- To compare the preoperative staging accuracy of rectal cancer using conventional EUS, 3D-EUS, and miniprobes.
- To evaluate the effectiveness of advanced EUS techniques in overcoming current staging limitations.
Main Methods:
- Sixty patients with rectal cancer were evaluated.
- Conventional EUS (7.5 and 12 MHz), 12 MHz miniprobes, and 3D-EUS (Easy 3D Freescan software) were utilized.
- Pathological assessment served as the gold standard for patients without preoperative therapy.
Main Results:
- 3D-EUS demonstrated the highest accuracy for T staging (96%) and N staging (100%), outperforming conventional EUS and miniprobes.
- Miniprobes showed significant agreement with conventional EUS for T staging (kappa = 0.81).
- 3D-EUS showed significant agreement with conventional EUS for T staging (k = 0.87) and superior N staging accuracy.
Conclusions:
- Miniprobes and particularly 3D-EUS show significant relevance for improving rectal cancer staging.
- 3D-EUS offers a highly accurate method for preoperative staging of rectal cancer, potentially enhancing treatment planning.
